All patients must please read and complete the Covid-19 consent form prior to their appointment. Please fill in and sign the form, and either print off or email to louise@acuherbs.co.uk. Please click on the PDF below which will allow you to do this:

In the eventuality that I (the practitioner) get symptoms of COVID-19 within 48 hours of having close contact with you during the appointment and then later test positive, I am obligated under law to provide your name, phone number or email and the date and time of your visit to the test and trace service. Please note that by attending the appointment you give consent for this.

The new contact tracing app has been introduced to help monitor the spread of coronavirus. We would be grateful if you would download the App via the link below prior to coming for your appointment. This will save time and you then simply have to scan the QR code in n reception before seeking the practitioner.. Thank you for your cooperation
